She doubles over.
That's what she was so upset with/angry at him about. That Rick allowed a situation where it wound up being Carl that put down "Walker Shane".

The dumb broad should have figured out that Carl would have to put down a zombie sooner or later. She should have been a real mother and kept better watch on him instead of letting him do as he pleases

Excellent point. Why not just climb up and toss the ladder aside? Or bring it up with you, they could have corralled more walkers that way.
I thought Rick's main reason for starting the fire was to try to draw the attention of all the walkers that were spread-out everywhere, not just the ones that were hot on his and Carl's heels.

But then he was stuck on the roof of a burning building.

Could, in all likelihood, be both. For every practical, blank-firing firearm on a movie set, there's a cast-resin duplicate that gets handled during most of the shooting. Sometimes a softer rubber replica as well, in case, for example, one the actors has to pisto-lwhip somebody, and the director doesn't want the stuntmen to step in. Notice how Rick's Python occasionally appears to have a satin finish rather than bright nickel? You're looking at the repro.
